Repeat Prescriptions
48 Hours notice is required for ALL repeat prescriptions.

Patients must ensure that requests for medication are submitted at least 48 hours before their medication runs out. Requests may be submitted :-

1. By post (enclosing a stamped self-addressed envelope)

2. In person using the repeat prescription request box in the surgery

3. By emailing rhoslansurgery@aol.com. Please identify yourself by giving your full name and address and date of birth when using this option.

4. Some local pharmacies now offer a prescription collection / delivery service.

5. By completing the form below.

We regret that, due to the inherent possibilities of errors occurring by this method, it is not possible to accept requests for repeat prescriptions by telephone.
